Key take away message – Recent Advances for Orthopaedic & Obesity Patients Care – HMAI Delhi CME

The HMAI Delhi State Branch, in collaboration with CK Birla Hospital, organised a highly informative CME on Recent Advances for Orthopaedic & Obesity Patients Care on February 4th, 2024, in New Delhi. The event featured insightful presentations by renowned experts:

  • Dr. Sukhvinder Singh Saggu, Director, GI Minimal Access & Bariatric Surgery, shed light on the latest developments in bariatric surgery.
  • Dr. Ashwani Maichand, Director, Orthopaedics, discussed cutting-edge advancements in orthopedic care.

The presentations were informative, clinical and stimulated a panel discussion on role of homeopathy in these areas ensued, featuring Dr. Maichand, Dr. Saggu, and esteemed senior members of HMAI Delhi, and interactive audience. The discussion delved deeper into the current clinical practice and trends and offered valuable insights for healthcare professionals.

Some of the key takeaways which I could note down can be summed up as follows:

Recent advances in Obesity

  • In overweight and obesity conditions – there are studies emerging on the role of thirfty gene involvement thus the treatment especially homeopathy involves taking care of the patient at genetic level. This can be achieved by using appropriate anti-miasmatic, intercurrent and deep acting medicines.
  • Obesity is not always about the energy expenditure, ie not about the calories we take or spent on our bodily activities.
  • Because of the easy food availability, we are turning our habits into addiction. We tend to eat more, exercise less and eat the food which is actually bad for our health.
  • The obesity is associated with the psychological issues. Therefore in such cases we need to have the medicines which affect the psychic of the patient.
  • Homeopathy has a number of medicine which acts on this line.
  • In obesity alone we can see the patient having inferiority, superiority, security complexes, low self esteem, depression etc. Therefore, in such cases we can use number of medicines such as Gelsemium, Natrum mur, Aurum metallicum and acid remedies.
  • Obesity is appended with the complex diseases and conditions from head to toe and can affect the quality of life in a very adverse manner in a patient,
  • When the BMI goes beyond 32.5 the body sets a metabolic point beyond which it does not shed weight. That is the reason why the patient does not lose weight easily despite eating healthy food or doing exercise on a regular basis, even if rigorous.
  • Bariatrics surgery can be taken case of excessive obesity but it is not the ultimate solution.
  • We need to understand that surgery is adjuvant and not curative in majority of the cases.
  • Despite undergoing surgery, the patient still needs to follow a healthy lifestyle
  • One important point, which was brought into light by Dr Saggu is that the complication of a surgery is usually less than the complications of obesity and co-morbid conditions.
  • Dr Saggu also discussed different types of bariatrics surgeries and their utilities in clinical practice
  • He could show us various research studies in which these surgeries could help patient from many points of view but the crux remains the same that the patient needs to follow a healthy lifestyle and work towards reducing the tendencies for add on or complex diseases.
  • Here homeopathy’s role comes in to play. We can use remedies like Calc carb, Capsicum, Natrum mur, Baryta carb, Phytolacca d etc

Recent advances in Orthopaedic care

  • Following Dr Saggu Dr Ashwini Maichand discussed various minimal invasive surgeries and minimal access surgeries in patients of orthopaedic diseases.
  • He brought into notice that there is an early onset of orthopaedic problems and because of such Minimal Access Ortho Surgeries (MAOS), surgeries are being considered safe effective and less troublesome now a days.
  • Especially the cases of avascular necrosis (AVN) hip and Ankylosing spondylitis are on rise because of usage of excessive medications and post Covid complications.
  • in such patients walking, sitting and performing day-to-day activities becomes a major challenge.
  • Therefore, with these kind of minimal invasive surgeries, we can offer patient a life which is more comfortable and easy.
  • He showed us many videos of his patients who underwent MAOS that he performed and the outcome of the surgeries were satisfactory.
  • Few of the clinical tips I would like to add here has told by Dr Maichand that in implants no cement should be used as it is cardio toxic in nature. Rather ceramic plants are preferred to be used.
  • These are safer and have more life than the conventional implants.
  • He also discussed various condition such as Lumbago Sciatica Syndrome, pain management in such cases and need of surgeries when it is indicated or not
  • Not every patient undergo or require surgery.

Homeopathic Discussion

  • Following these two presentations there was a detailed panel discussion on the role of Homoeopathy in obesity and orthopaedic related problems.
  • It was an open house discussion where we discuss about the curative, management and recurrence aspect of cases, specially in preoperative and post-operative surgical cases.
  • In cases of surgery, the Homoeopathy has a major role to play. There is a need to integrate use of Homeopathy in such cases for not only their physical but also mental well-being.
  • For such instances such as tissue damage we have remedies like Hypericum, Calendula, Echinacea and Symphytum.
  • Also when it comes to pain management homoeopathy can be given as adjuvant or add therapy to relieve the pain without giving any side-effect to the patient.
  • There are couple of remedies which are used such as Radium brom, Arnica, Ruta, Bellis p, Berb vul, Sabina etc.
  • For pre or post-surgical mental stress we can use Arnica, Syphillium and Staphysagaria etc.
  • For excessive use of x ray radiations – we can use Radium brom, x ray Natrum mur etc
  • It was discussed that Colocynth, Arnica, Natrum phos etc can be considered as good remedies for pain management.
  • In pre-or post-operative cases where we have acidity like complaints or GERD we can use remedies like Natrum phos, Robin Natrum phos, Sulphur etc can be used.
  • Our remedies work on the dynamic level therefore they can be safely given along with the conventional therapy
  • There is need to understand the relation of the medicine at the dynamic level so that they can be easily admitted in the patient and also we need to document such cases in our practice for more confidence and faith in Homoeopathy.

The CME concluded on a positive note, with participants expressing their appreciation for the clinical learnings and knowledge gained.
