Clinical Updates in Medical Hepatology and role of Homeopathy

Below are the excerpts of the homeopathic discussion during CME on Clinical Updates in Medical Hepatology, held in collaboration with Max Hospital Dwarka and The Homoeopathic Medical Association of India, Delhi State Branch on 16th Feb 2025.

Important points to consider:

  • As Liver is one of the largest and vital organs of the human body every deviation in its structure and function has an underlying cause.
  • We must aim to find the derangement, their cause, risk factors and maintaining causes as well.
  • In majority of the cases, simple investigations such as Liver function test, Ultrasonogram of the abdomen, are sufficient for the follow ups.
  • When needed or case in suspicion, we must take help of the experts and specialised investigations such as CT / MRI scan, Fibroscan etc.
  • Specialized investigations should not be repeated too often, especially when not advised or needed.
  • In addition to standard Diagnosis + Management + Prognosis, we can use homeopathy in majority of liver ailments.
  • The symptomatology is very important as it gives us direct hint to deal an “individual” suffering from liver derangements.
  • At times, we also encounter individuals consulting us for their liver / blood’s “altered states” with no or minimal symptoms.
  • Get the investigations repeated if the liver indices seem erroneous or are not corroborating with the clinical picture.
  • In cases where symptoms are vague / common we can use the targeted remedies in addition to individualised / constitutional remedies.
  • If there are no symptoms, we should not repeat the doses too often, rather we can give moderate to high potencies of the indicated remedy and follow the patient fortnightly / monthly along with investigations.

Tincture vs Dilutions vs Pills

The discussion started with points on Potencies vs Tincture in cases of Liver disorders.

Few months back there was backlashed with a published article which claimed that homeopathy induced alcohol cirrhosis of the liver in a patient. To this, an open letter was published by (Ref – https://sauravarora.com/homeopathy-adverse/) in which it was discussed that why this is not possible to induce liver cirrhosis with use of homeopathic medicines in moderation and under guidance. Some of the points can be summed up as:

SCENARIO – when a patient with liver related complaints is treated under supervision by a registered homeopathic physician using cardinal principles of homeopathy only.

  • The usual repetition of doses of homeopathy medicines are 2-3 times a day, or even less when constitutional medicines are prescribed.
  • If a patient is given medicine in liquid form – on an average, a 30 ml of tincture / dilution contains around 900 drops (as per standard dropper used in routine clinical practice).
  • This means each drop is equivalent to 0.03 ml and thus each drop should have = 0.03ml x 20% (in case of tincture) = 0.006ml of alcohol or 0.03 ml x 70%(in case of dilutions)  = 0.026 ml of alcohol.
  • For syrups the alcohol content for each ml is 10% volume by volume, it for each ml the alcohol content is 0.1ml
  • If the patient is taking 10 drops thrice daily (as common dose) it means he is consuming 0.006 ml x 30 drops = 0.18ml alcohol every day (in tinctures) and 0.8ml (in dilution).
  • For syrups even if the patient takes 3.5ml as one dose and two doses a day ie 7ml of syrup, the alcohol consumed per day is 0.7ml only

PS: The article published claimed that patient took medicine having “18%” of alcohol, So, it is still debatable that this quantity of alcohol is sufficient to induce cirrhosis of liver.

There might be instance where a patient can misuse homeopathic remedies without consultation and prescription for which homeopathy should not be blamed without any verifiable evidence.

FATTY LIVER – Liver Enlargement

  • Liver enlarged – Andrographis, Ars alb, Calc ars, Carduus marianus, Chelidonium, China, Digitalis, Kali bi, Lycopodium, Mag mur, Myrica, Taraxacum
  • High livers – Allium sativum (in alcoholics)
  • Good livers – Berb vulgaris
  • Swelling of Liver – Dolichos
  • Atrophy – Phosphorus (an important remedy)
  • Chronic liver ailments – Selenium

Raised Enzymes

Along with general remedies for liver we can use

  • Kalmegh
  • Hydrastis

Liver destruction

  • Cirrhosis of Liver – Kali bi
  • Early Cirrhosis: Senecio
  • Cirrhosis and Sclerosis of Liver – Aurum met
  • Cirrhosis of liver with ascites – Quassia Amara
  • Vomiting of bile in cirrhosis of liver – Curare
  • Hob-nailed liver of alcoholics – Acidum flouricum
  • Cancer pains – Acidum flouricum, Hydrastis can
  • Degeneration of Liver due to diabetes – Uranium nitricum

Infections

  • Hepatitis (A) – Acid nit, Chelidonium, Myrica, Lycopodium, Vipera
  • Typhoid – Soreness – Baptisia
  • After alcohol – Lachesis

Gastric symptoms

  • Hunger associated with Liver Ailments
    • Increased: Abies can
    • Decrease: Three hours after meals – Aesculus
  • Gastric disturbance with sharp pains in liver region – Agaricus
  • Fullness as in Portal Hypertension – Aloes
  • Torpidity of the liver; portal engorgement
  • Stitching pains – Berb vulgaris
  • Pains – Calcarea sulph, Carbo veg, Chelidonium
